Please complete this form to book a place on our ‘UV MULTI SPORTS After School Club’ commencing Tue 4th June until Tue 9th July 2024.
* Pupils in Years 3-6 may attend, please note PLACES ARE LIMITED.
* A message will be sent out via Dojo to inform you if your child has a place.
* Important: Please could children bring their own PE Kit from home for clubs.
* Children will not be permitted to walk home alone and should be collected from School Gate 1 (Nursery) at 4.15pm promptly.
