Please complete this form to book a place on our Multi sports clubs which will be held during Autumn Term 2. The club will run from Wed 5th November until Fri 19th December 2025, from 3.15pm until 4.10pm, for 7 weeks, as follows:.

* Wednesday – Year 1, Year 2
* Thursday – Year 3, Year 4
* Friday – Year 5, Year 6

* DF Next Level Coaching will be running the club.
* The cost per session is £1.00 – (Total cost £7.00 for 7 weeks, to be paid via Arbor, following confirmation of a place).
* Pupils in Year 1 to Year 6 may attend, please note PLACES ARE LIMITED.
* A message will be sent out via Dojo to confirm if your child has secured a place.

IMPORTANT:
* Please could children bring their own PE Kit from home for the club.
* Children will not be permitted to walk home alone and should be collected from School Gate 1 (Nursery) at 4.10pm promptly.
